Permission for Installation
I authorize representatives of _______________ to work on my computer (PC) in accordance with the work order numbered above. Installation of necessary applications and drivers can be installed on my PC. My PC can be operated for the purpose of testing, troubleshooteing problems and inspection with me accepting any risk. The representatives will not be held responsible for losses caused by fire, theft, accident or any other cause, It is also undersood that the representatives of _______________ shall not be held responsible for any loss of data that on the hard or floppy driver of my PC.
(adequate procedures for data backup should be implemented by the owner of the PC to facillitate reestablishment of data and applications if necessary.)
I nderstand that is it an integral part of this installation and the normal operation of a PC to make regular data backups. nevertheless, the representatives of ______________ will not make a backup of data or application of the main hard drive of my PC while making the installation of the Satellite modem and I authorize them to do so without becoming responsible for possible losses to data, drivers or any apllication of accessory in my PC.

any loss that may occur.
________ I understand that an Internet access system using a Satellite modem for reception to be
installed in my PC has certain minimum PC and other capacity requirements. I will not
hold ____________ responsible if my PC ends up not meeting those minimum
requirements. This may include the non-refundability of my installation costs.
I understand the above and give permission for the installation of the Satellite modem in my PC.
